If you are a new patient, please download, complete these forms and email them to me, jenbuerger@gmail.com, 24 hours prior to our initial appointment so I can review them before we meet. Please also bring in a printed copy of the completed forms that can be put in your file. If you are unable to complete these forms, please come 10 minutes early to complete them in my office at our first appointment. These forms are necessary for us to work together.
